
Forms & Waivers Must Be Signed
After booking your appointment, you’ll see a notification on the confirmation page reminding you to complete all required forms and waivers. You’ll also receive reminders via text and email. To complete the forms and waivers, simply select “Manage Appointment” in your confirmation message or email. Please note: If forms and waivers are not completed, your appointment will be canceled. This ensures we can provide safe and timely service to all clients.
